Diabetes PHD - Personal Health Decisions  Web-based Tool to Assess Risk for Diabetes and for Diabetes-related Complications

The American Diabetes Association has a new interactive web-based tool that makes it easier for people with diabetes -- and anyone at risk for developing diabetes, heart disease, or stroke -- to better manage their health. Diabetes PHD (Personal Health Decisions), a powerful new risk assessment, is available for use by consumers at no charge. In addition to assessing risk, it can be used to explore the effects of a wide variety of health care interventions, including losing weight, stopping smoking, and taking certain medications.

National Diabetes Education Program

The National Diabetes Education Program is a federally funded program sponsored by the U.S. Department of Health and Human Services' National Institute of Health and the Centers for Disease Control and Prevention and includes over 200 partners at the federal, state, and local levels, working together to reduce the morbidity and mortality associated with diabetes.
